Validation forms and permissions have been sent home with students. Parents/guardians are asked to make any amendments, mark the permissions yes or no, sign and return the forms back to the office or class teacher as soon as possible. It is important we have up to date information for each student, especially in relation to emergency contacts. Parent/guardian information is listed on the forms but other emergency contacts are required to be added each year.

The completion of the Validation form is a mandatory requirement by the Department for Education, Children, and Young People and must be done on an annual basis.  If validation forms are not returned, students may miss out on participating in school excursions.

We understand that the forms can be time consuming, and we appreciate your efforts to return them ASAP. The first class to have all forms returned will win an icy pole treat day for their class!
